How to Apply
After you’ve satisfied the prerequisites, please download the application package from the link below.
- Follow the checklist provided to ensure you submit a complete application. Incomplete applications will not be considered.
- Fill out the application – if you’re a CSU/OSI or SCMI consortium member student, please include your faculty referral. Make sure to include copies of your certification card(s) when you submit it.
- Fill out the waiver – make sure it’s witnessed (this can be anyone).
- Complete the medical history and authorization to release medical information. Your dive medical exam may be conducted by a Physician’s Assistant or Nurse Practitioner, but must be signed by a Medical Doctor (MD). If you have an existing medical condition that might impair your ability to work as a research diver, you may be required to have your exam conducted by an MD specializing in dive medicine. Make sure your report is signed, dated, and the duration has been checked.
- Make sure you have everything on the required equipment list. We have a limited supply of equipment we can loan to students on a first-come-first-served basis. If you have questions or need to borrow gear, make sure you contact us as soon as possible.
- If you own dive equipment or plan to purchase it before the class starts make sure the service forms have been completed – review them carefully prior to submitting your application.
Please note: CSU/OSI applicants qualify for equipment discounts – these are signficant. Contact the DSO for more information.

COVID19 DIVING AND BOATING NOTIFICATION
The Campuses of the California State University, have adopted federal and state guidelines regarding COVID mitigation protocols. In the event of an outbreak these guidelines and supplemental protocols may be implemented and will impact locations and scheduling for the AAUS course.
